How do I submit my Lost Winning Ticket case?

Step 1: Digital Submission

1. Please ‘Submit a case’ in your MyMagnum 4D app under ‘Lost Winning Ticket Plan’.

2. You are required to submit:

  • A photo of your National Registration Identity Card (NRIC) (front & back) for Malaysians or Passport for non-Malaysians

  • Scanned copy of the police report lodged by you (stamped and signed by the authority)

  • Selection of your lost winning ticket

  • Your bank account details

 

Step 2: Physical Submission

1. You may submit your physical documents for your case at any Magnum 4D State Offices. Kindly locate the nearest Magnum 4D State Office here:

2. You are required to submit:

  • Police report lodged by you (original valid copy, stamped and signed by the authority)

  • Photocopy of your National Registration Identity Card (NRIC) (Front & Back) for Malaysians or Passport for non-Malaysians

  • Original NRIC or Passport

  • You must show your registered M Premium account via MyMagnum 4D App installed in your device
